Your community hospital

Built in 1999, Charles A. Cannon, Jr. Memorial Hospital is a 25-bed acute care hospital with a 10-bed inpatient behavioral health unit. The full-service emergency department, imaging department, laboratory, outpatient behavioral health, The Rehabilitation Center offering physical and occupational therapies, and cardiopulmonary rehabilitation round out the list of services.

Conveniently located in Linville, the center point of Avery county, Cannon is fully accredited by the Joint Commission for the Accreditation of Healthcare Organizations. Cannon Memorial Hospital was designated as a Critical Access Hospital in December 2005 to ensure long term operational and financial viability and to better extend services to the local community.


Hospital Renovations

My son had to go to the ER yesterday. The wait time was short. He was pleased and said the doctor was very helpful. Very grateful to Dr. Wilcox and the staff for taking such good care of him!

The outpatient behavioral health program of Cannon Memorial Hospital is designed to meet the needs of adults, children and families experiencing a variety of problematic behaviors, thoughts and life patterns. We work to improve emotional stability and increase general functioning, as well as help clients identify, develop and increase the use of effective coping skills by emphasizing the existing strengths of the individual or family system.

Through an initial screening and assessment process, our staff, in collaboration with clients, work together to determine the most appropriate treatment for each individual and their family.

Medication Management

Outpatient Behavioral Health at Cannon Memorial Hospital is now accepting new patients for medication management. Our team of physicians and psychiatric mental health nurse practitioners have years of combined experience in behavioral health treatment and our goal is to provide the best psychiatric care in the High County.

Patients are provided with customized, therapeutic interventions to meet their individual needs. The unique locations allow patients access to strength and conditioning equipment and pool at The Williams YMCA of Avery County and the Paul Broyhill Wellness Center in Boone, providing a continuum of care from acute care to wellness programming. Some Outpatient Rehab Services require a physician’s referral for insurance coverage. Please contact the Rehabilitation Center for more information about referrals.

The Rehabilitation Center therapists are trained and licensed to administer WorkSTEPS functional employment testing for your employees. We offer these services at our Boone location.

Patient Information


Most insurance plans still require proof of medical necessity to reimburse for rehabilitation services. For that reason, it is prudent for patients to have a referral (prescription) for physical and occupational therapy services. We ask that physicians fax a referral to our office, or send it directly with their patients for the initial evaluation.


Please contact our office managers, in Boone at (828) 268-9043, ext. 4 or in Linville at (828) 737-7520, with any questions regarding referrals and insurance.


As a courtesy, we will bill the patient’s insurance for the therapy services provided. Because we are a hospital-based facility, insurances view us differently than private practice providers. In most insurance plans, charges for outpatient therapy services will go towards the deductible.

It is the patient’s responsibility to determine if Cannon Memorial Hospital or Watauga Medical Center is in-network with your insurance plan. When inquiring about whether or not we are “in network” with your insurance company, please refer to us as Cannon Memorial Hospital or Watauga Medical Center. We can assist you with this process, if you have any questions.

Payment Arrangements

Patients will receive a bill from Cannon Memorial Hospital (Linville location) or Watauga Medical Center (Boone location), for payments due. If payment in full is not possible, a Patient Account Representative must be contacted to make payment arrangements. See contact information below.

Financial Assistance

Cannon Memorial Hospital and Watauga Medical Center provide financial assistance for patients in need. Verification of income and assets is required. To apply for financial assistance please call:¨Financial Counselor at (828) 262-4110. To be considered for financial assistance you must submit an application within 60 days of discharge from physical therapy.

I had been experiencing benign paroxysmal positional vertigo (BBPV) for a month and had one Epley maneuver when I saw Katherine Graham. She is a godsend! She thoroughly explained what was happening to me and performed two Epley maneuvers back-to-back. The next day I woke up without feeling dizzy and I felt as though I had my life back. Thank you!


I am so thankful for the dedicated, knowledgeable, and friendly staff at The Rehabilitation Center.


I have been extremely well-treated during my therapy. Melody was very accommodating with appointments. Michelle Forrest provided methods very appropriate to my condition – very basic at the beginning, but more applicable to my condition as time passed. But above all, she was very patient with me.


The service was excellent. Everyone was courteous. The therapist was wonderful in working with me and explaining everything to me. I could not have made it without these services. Thank you.


I just want to say how awesome the PT workers are! They care above and beyond for their patients. Susan and A.J. are so wonderful. I went from using a walker, wheelchair, and cane to walking with nothing! The workers and the therapy pool have had a miraculous effect.


Michelle Forrest is excellent and I am here to sing her praises. Today was my last session for treating lymphedema as a result of breast cancer radiation and lymph node removal. She was professional and also caring. I appreciated her attention to detail, her love for her profession shows greatly.


My time at the Wellness Center was incredible. It was a long road to recovery and I am so thankful for my PT therapist Susan Curtin and my trainer Jaco Gerbrands.
