Moving Forward with Diabetes: is a three-month program for patients with T1 or T2, gestational diabetes or glucose intolerance. Individual assessment, group educational classes, diabetes educator (CDE), and physician follow-up.

Medical Nutrition Therapy
Individual nutrition counseling, with a qualified registered dietitian/nutritionist (RDN), is available for patients with T1, T2, or gestational diabetes. Medical nutrition therapy (MNT) can help individuals with diabetes improve their blood glucose, body weight, blood pressure and lipids (cholesterol, triglycerides). Referral is required.
